Integrating Health Coaching Into a Comprehensive Health Management Effort, a November 15, 2006 audio conference on CD-ROM, provides an inside look at how health coaching programs can best be integrated into an overall health management strategy.

Click here to listen to some pre-conference comments from our audio conference presenter.

Health coaches are becoming an increasingly popular tool as healthcare organizations seek to improve the health of their members with chronic health conditions. But health coaches must be integrated into an overall health management strategy to be effective. Moving patients across the continuum from health coaching to other self-management initiatives should be seamless to increase its effectiveness.

During this 90-minute audio conference on CD-ROM, Dr. Susan Butterworth, director of health management services at Oregon Health & Science University and Roger Reed, executive vice president for market operations, Gordian Health Solutions, share strategies on how to effectively integrate coaching into a health management program.

You will get details on how to:

  • Move patients in and out of health coaching as part of an overall health management strategy;
  • Evaluate a patient's need for health coaching versus other forms of health management;
  • Recruit and engage members in health coaching and health management programs;
  • Create a coordinated approach when part of a health management program is outsourced; and
  • Integrate data from all sources -- claims, health risk assessments and prescription drugs -- to make appropriate referrals for health management programs.

You can "attend" this program right in your office and enjoy significant savings Ė no travel time or hassle; no hotel expenses. Itís so convenient! Invite your staff members to listen to the CD.
