An innovative Care Coordination Center (C3) at Ochsner Health System, under the Ochsner Accountable Care program, is charged with both clinical and non-clinical care coordination for the nine-hospital system. From managing in-bound calls to a nurse advice line and making outbound calls for post-discharge care transitions and care gap management, this system-wide approach was launched in July of 2013 as the organization hit a tipping point of patients for which they are at financial risk for medical management.

    Mark Green, is the Director of Ochsner's Care Coordination Center (C3), a corporate resource housed under Ochsner's Accountable Care Organization. The Care Coordination Center has the responsibilities to develop, implement and monitor transitional care management across the healthcare continuum for the system.

    In his previous roles, Mr. Green functioned as a system strategic program manager, as well as a corporate Lean Six Sigma resource. With a clinical operations background in diagnostic imaging, Mr. Green has over 25 years of hospital, clinic and corporate operations knowledge that he brings with him.

    Mr. Green holds an undergraduate degree in radiology sciences (BSRT), an MBA from Marylhust University, and holds certifications in both project management (PMP) and Lean Six Sigma. He has presented nationally on project management and Lean Six Sigma in the healthcare industry.
