During Dual Eligibles: Closing Care Gaps and Engaging Members in Self-Management, an October 2, 2013 webinar, now available for replay, Pamme Taylor, WellCare’s vice president of advocacy and community-based programs, shares Wellcare’s strategy for meeting a broad spectrum of a member’s needs with community-based services and how these partnerships contribute to the engagement of duals in self-management of their care.

  • Connects with primary care physicians and specialists to assess member needs;
  • Uses PCPs and specialists to identify cultural and ethnic preferences to improve member engagement;
  • Developed a referral tracking system embedded in the electronic health record for case managers to link members to available services;
  • Expanded the referral tracking system data inputs to create a truly population health-focused platform; and
  • Is working with community-based programs with constrained budgets to sustain needed services for their members.

Taylor also shares the impact of these initiatives on emergency room utilization, quality scores and gaps in care.

Pamme Lyons Taylor is the vice president of advocacy and community based programs at WellCare Health Plans, Inc. She joined WellCare in 2011. Taylor is focused on connecting managed care with public health to leverage and sustain the network for social safety net services across the country.

Taylor utilizes her experience with government-sponsored healthcare programs to connect people in need with health and social services in their local community. She has more than 15 years of experience in the managed care industry. Previously, she held community advocacy leadership roles at LA Care Health Plan, UnitedHealthCare Community and State, Health Net and PacifiCare Health Systems.

Over her career, she has convened more than 20 different collaborative initiatives with more than 400 local and national organizations. Most recently, she assembled The CommUnity Commitment for WellCare, with more than 5,000 participating organizations.

Taylor has degrees in communications, public relations, business administration, healthcare administration and computer programming. She is currently working on her PhD in public health with an emphasis on community health. Taylor was recently selected to join Leadership Florida, and she is an active board member for several community-based organizations across the United States. Taylor is profoundly hearing impaired and works closely with several national disability-focused organizations to advocate for increased employment opportunities for individuals with disabilities and other diverse cultures.
