Using a self-developed approach that combines elements of Geisinger’s Proven Health® Navigator, Johns Hopkins Guided Care Nursing and the Wagner Chronic Care Model, Taconic Professional Resources is assisting physician practices in the New York Hudson Valley to improve population health and care for their sickest patients through the use of embedded RN case managers.
Taconic supports, manages, trains and evaluates embedded case managers for physician practices. It started with an 18-month pilot program and moved into an expanded model that serves the CMS Innovation Center’s Comprehensive Primary Care (CPC) initiative—as well as physician practices that function in medical home and ACO structures.

Annette C. Watson, RN-BC, CCM, MBA, senior vice president of community transformation for Taconic
IPA, brings more than 25 years’ experience in healthcare executive leadership focused on quality improvement and cultural transformation. Over the course of her career, she has demonstrated a commitment to improving healthcare quality through innovation and collaboration. She is a vocal advocate of health information technology adoption, interoperability and transparency in order to promote quality, cost effective care, balancing the need for patient privacy and security.
Watson is the founder and principal of Watson International Consulting, a privately held consulting firm specializing in the effective design of strategies that result in health and human service organizational performance improvement consistent with the IOM Six Aims and the IHI Triple Aims. Recent clients have included the U.S. Department of Defense/TRICARE, the Disease Management Association of America and the Association of Community Cancer Centers.
Previously, she served as managing director of global emerging business for CARF International, and before that as senior vice president at URAC, where she was also the organization’s first chief accreditation officer. Prior to entering the accreditation field, she held executive, management and clinical positions in the hospital, home health, family practice, insurance and managed care settings.
Watson is immediate past chair of the Commission for Case Manager Certification; she has served as a Commissioner since 2007. She is a long-term member of the Case Management Society of America (CMSA) and was a founding member of the New England chapter. She recently served on the CMSA National Standards of Practice Committee and the Governance Task Force.
Watson has served on numerous national committees and task forces, including the National Quality Forum's Research and Quality Council, the National Business Coalition on Health's Healthcare Leadership Council, the Disease Management Association of America’s Patient Safety Committee, the Academy for Health Care Management/Curriculum Advisory Committee and the American College of Medical Quality Forum on Quality in Managed Care.
