As Health Care Services Corporation rolls out care coordination programs for dual eligibles in select markets across the country, it is encompassing a broader scope of care coordination beyond just a medical model.
Listen to pre-conference comments from Julie Faulhaber.
During Moving Beyond the Medical Care Coordination Model for Dual Eligibles, a March 12th webinar now available for replay, Julie Faulhaber, vice president of enterprise Medicaid at Health Care Service Corporation, shares her organization's approach to designing a care coordination model for dual eligibles and initial findings from these new programs.
You'll learn how HCSC:
- Stratifies, finds and engages dual eligible beneficiaries into care coordination efforts;
- Utilizes either a nurse-led or social worker-led care coordination strategy based on the unique needs of the population served;
- Supplements its own in-house care coordination resources with community based services;
- Has created unique models to serve the distinctly different needs of dual eligibles who have serious mental illnesses; those who have physical disabilities; and those with developmental disabilities; and
- Has overcome the roadblocks it has encountered in the dual eligible market.
