As part of its population health management strategy, Massachusetts General Hospital uses a multi-disciplinary team to address high-risk and rising-risk patients.
By addressing both risk levels, the 999-bed hospital, ranked as the #1 hospital in the nation by U.S. News & World Report® based on its quality of care, patient safety and reputation in 16 clinical specialties, has been able to close gaps in care, reduce high-cost healthcare utilization and improve provider performance measures.
During Targeting High-Risk and Rising-Risk Patients: A Multi-Pronged Strategy, a 45-minute webinar on August 1st, now available for replay, Dr. Adrian Zai, clinical director of population informatics at Massachusetts General Hospital, shares the key details behind his organization's strategy and results they've achieved.
You will learn:
- Why Massachusetts General adopted the multi-pronged approach to address high-risk and rising-risk and how this strategy reduces the number of high-risk patients among a defined population;
- Which conditions the hospital targets and how Massachusetts General layers its interventions to include multiple touchpoints for optimal impact;
- How to determine which data points to collect for effective, financially feasible interventions targeted at both high-risk and rising-risk patients; and
- The role of technology in population health management and how to tightly connect the technology to workflow to maximize the interventions' impact.
